Medical Forms

  • BSA Medical Forms - The Boy Scouts require individual troops to maintain an authorization for medications and emergency treatment for each scout. The form must be completed yearly along with Boy Scout medical forms A, B and C.  Please complete all parts of this form and return it to the Troop 1128 Medical Records Coordinator.
  • Authorization to Monitor Medication and Consent to Emergency Treatment. - The taking of (1) prescription medication and (2) non-prescription medication is the responsibility of the Scout. The Scout may self-medicate, or one or more Troop adult volunteer(s) may monitor the Scout's taking of medication. Should the Scout's parents request in writing, using this form, one or more Troop 1128 adult volunteer(s) will make good faith efforts to monitor the Scout's taking of such medications

 Miscellaneous Forms

  • Advancement Green Sheet - Print this form and complete the top section. Scouts should see the Troop Advancement Coordinator when they are ready to advance in rank.
  • Eagle Advancement Gold Card - This form outlines the special steps to be taken before an Eagle Board of Review.
  • Quartermaster Checklists - Here is an Excel file with three tabs containing checklists to help keep track of troop gear.
  • Driver Information - To protect our BSA Insurance Policy, we need current information on all drivers transporting scouts. Please print and complete this form. Then give both pages to any member of the Troop Committee.
  • Photo Release - Please print this form, sign the appropriate section, and return it to any member of the Troop Committee.
  • Service Record Form - Use this form for recording volunteer service hours donated by scouts and adults. It should always be completed and returned for troop activities that provide community service. Examples include:
    • Eagle Service Projects
    • Scouting for Food
    • Community service activities included as part of other troop events or campouts

  • Youth Protection Training - All Scouters who interact with scouts by virtue of their position, (e.g., Assistant Scoutmasters, Merit Badge Counselors) must complete BSA Youth Protection Training. One way of meeting this requirement is the online Youth Protection course.

  • Merit Badge Info - The U.S. Scouting Service Project sponsors this excellent source of current merit badge information.
  • Merit Badge Counselor Info - Information and application form to be a merit badge counselor.
  • Philmont Scout Ranch - Information on the premier High Adventure destination.
  • King Eagle Scout Scholarship - The Sons of the American Revolution runs a scholarship program for Eagle Scouts. The application may be downloaded from their website.
